Insurance
Bodies in Balance accepts all insurances. As a courtesy for our patients we will contact your insurance carrier to determine your benefits for physical therapy and submit all claims to your carrier. Additionally the patient should also contact their insurance carrier to know their physical therapy benefits and be aware of all covered and non-covered benefits. If your carrier denies treatment for ANY reason, you acknowledge a discounted fee of $70/visit for services received.
Patients are responsible for:
(1) Insurance deductible and (2) co-insurance or co-payments per office visits.
At the time of your first visit please have:
(1) Current insurance card, (2) Picture I.D. and (3) Prescription for Physical Therapy from a referring doctor.
